colonial (lower case) lighthouses is a description given in many books to the lighthouses built prior to the formation of the US and which were taken over by the USLHE IN 1789. There, of course, was no colonial agency.

Cape Cod is not correct as it was not established until 1797.

I've misplaced my book with the complete correct information, but I believe the others are correct. Paul is correct in that Newburyport was built in 1788 and I believe it was one of the 12, although not actually "ceded" (slow on the paperwork I expect) to the USLHE until 1790 according to another source I have. Cape Ann is definitely correct as it dates to 1771.

The only one that was missed is Plymouth, MA or Gurnet which was established in 1768.

Inasmuch as Roland provided the most information, he should probably go next unless he wishes to defer to Paul.
